Statement of Release

1. I/We, the parents/guardians of the above named child, hereby give my/our approval for his/her participation in any and all BNBLL activities including transportation to and from the activities.

2.  I/We hereby waive, release, absolve, indemnify, and agree to hold harmless BNBLL, the organizers, sponsors, supervisors, participants, and persons transporting my/our child to and from activities from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause.

3. I/We will furnish a certified birth certificate of the above named child to BNBLL.

4. I/We will provide proof of legal residence (as defined by Little League Baseball, Inc.) to BNBLL.

5.  I/We agree to return upon request or at the end of the season any and all equipment issued to my/our child in as good condition as when received except for normal wear and tear.

6. I/We will participate in all BNBLL fundraisers and turn in all money by the assigned due date. If fundraiser obligation is not fulfilled, I will honor the disciplinary action of BNBLL.

7.  I/We acknowledge that to the best of my knowledge, information and belief, the above named child is physically and mentally able to participate in BNBLL programs with any undue or unusual risk to him/her or to others
